Pharmacy Residency Application The applicant must provide a letter of intent, curriculum vitae, pharmacy school transcript and three letters of recommendation. A personal interview is required of all candidates. Applicants must hold a Doctor of Pharmacy degree from a school of pharmacy accredited by the American Council on Pharmaceutical Education, and must be eligible for licensure in Missouri.
