To request a copy of your personnel file please:
Fill out the request form
Bring a positive photo ID
Have $10.00 (cash only) for processing
Bring all of the above to Human Resources at the University Hospital located at 1 Hospital Drive (1W-46), Columbia, MO 65212 (Next to Boone Co Bank).
The employee will receive their personnel file in PDF format via their MUHC email account.
