Missouri Rehabilitation Center accepts a wide variety of diagnoses in the areas of pulmonary care, medically complex care, wound care, neuro/post-trauma care and rehabilitation services. As a long-term acute care facility, Missouri Rehabilitaiton Center only accepts referrals directly from physicians.

Missouri Rehabilitation Center has four admissions liaisons that cover the state of Missouri. We conduct on-site screenings whenever possible within 24-48 hours. The liaisons work closely with physicians, discharge planners and case managers to help identify patients who may benefit from our services.  Staff may meet with the patient and/or families to provide information about our services or facility to enable them to make the best decision for their care. Staff is also available to answer questions regarding financial and insurance questions.

When making a referral, please have the following information available so that your request can be processed efficiently: 

  • Patient name
  • Date of birth
  • Person inquiring and phone number
  • Face sheet from referring facility
  • Copy of insurance card, if available
  • Name of referring physician and phone number
  • History and physical
  • Consults
  • FIM notes for rehab patients
  • OP reports
  • Labs/MRI/CT reports
  • MAR/Med list
  • Height, weight and code status
