Pediatric Injury Prevention & Safety Outreach Program works with community organizations such as the Voluntary Action Center to help identify families who are eligible for no-cost injury prevention essentials. The Kohl's Safety Store is funded by the gift from Kohl's Department Store specialty merchandise.

Are you eligible for Kohl's Safety Store products?

If you are interested in receiving Kohl's Safety Store products at no cost to you, please visit the Voluntary Action Center for eligibility screening. For the eligibility screening, you will need to provide the following:

• Proof of residency
• Your child's birth certificate
• Guardian's personal identification
• Proof of income

If you are eligible, Voluntary Action Center will provide a voucher that will be needed for your appointment at the Kohl's Safety Store.
