Who May Sign an Authorization Form?


In Missouri, the only persons authorized to sign an authorization for medical records are:

  • Persons 18 years of age or older. If the patient is a minor (less then 18 years of are), the authorization must be signed by a parent or legal guardian of the patient.
  • If the patient is deceased, an authorization must be signed by the executor of the patient's estate.
  • If the patient is unable to sign by reason of physical or mental incapacity, the authorization must be signed by the patient's legal representative.
