Aging in Place
The Aging in Place program is designed to help clients stay in the home of their choice for as long as possible. Participants receive care to prevent or postpone the need to move to a more restrictive living environment like a nursing home. In this program, a skilled nursing assessment is done on admission and every six months which includes a fall risk assessment. There are also a number of home care visits that participants may use at the time of their choice.
As clients care needs increase, they may contract for additional services with Sinclair Home Care to help them remain independent. These services are offered at TigerPlace and Lenoir Woods in Columbia, Missouri.
The Aging in Place program is one of three state sponsored aging-in-place evaluations. Preliminary research shows that the program does positively impact the lives of the participants.
