Referrals
To make a referral for medical equipment or home health services, please call 884-7000. For fast telephone referrals, please have the following information ready.
- Name
- Address
- Telephone number
- Date of birth
- Social security number
- Insurance information
- Diagnoses
- Physician's name and telephone number
- Referring physician and primary physician (if different)
- Treatment plan (what disciplines will be involved in the clients care)

Please note: Physician orders are required for any treatment that is provided under Medicare, Medicaid, or insurance.
Referral Checklist/For Physicians
A home health referral should be made if the patient
- Needs Skilled Nursing for:
- Treatments continued at home (infusions, catheters, ostomies, wound care, oxygen, etc.)
- A new diagnosis or exacerbation of an illness (ex. CHF, COPD, MS, etc.)
- Continued teaching at home
- New medications, changes in medications, or multiple medications
- Needs physical, occupational, and/or speech therapy
- Is homebound (if even briefly following hospitalization)
- Homebound patients may still attend religious services, keep medical and hair appointments, etc.
- Has had frequent hospitalizations
- Is any age and frail
- Is over 65 years old
