New Admissions Process
Physician involvement is crucial to the new service. Immediately after accepting an admission, physicians are asked to submit a request for admission form, complete with the patient's name, diagnosis, presenting signs and symptoms, proposed treatment plan, attending physician, bed location preference, contact name, telephone number and primary-care or referring physician. From this information, the center's staff members will:
- Complete the request for admission form
- Review the diagnosis, clinical status and treatment plan to be implemented against InterQual criteria utilization management standards used nationwide to help assist with the appropriate placement of patients
- Oversee completion of insurance verification
- Complete timely precertifications
- Notify physicians of any admission denials so that timely appeals can be made
- Coordinate alternate levels of care
- Assess high-risk admissions for referral to Social Services and utilization-management nurses
Care Coordination Center streamlines admissions
