Medical Records

We keep all the documents and records organized and available for your physician’s review. We ensure the documentation of your care is appropriate for federal and state guidelines and billing purposes. We make sure copies of the records that document your health care are available for any physician you choose, your insurance company, your attorney or for your personal use.

What Are Medical Records?

A medical record is documentation of medical and health care services provided to a patient at the directions of a physician, nurse or other caregiver. These records include documentation of medical observations and opinions by these professionals as well as the treatment and diagnostic documentation, such as X-rays, EKGs and laboratory work. These records can be a paper record, electronic record or both. The medical record documenting your health care is the property of MU Health Care.

Access of My Medical Records

University of Missouri Health Care’s Health Information Services Department maintains a federally compliant Release of Information Authorization form. This form identifies to whom we are authorized to release information and what records you want released. You will need to complete this authorization form in the department. You may come to the department and sign the form. If it is more convenient, we can fax the form to you and you can fax it back when it is completed.

Federal regulations impose further requirements in accessing information on drug and alcohol abuse medical records and mental health medical records. Specific detailed authorizations must be obtained from the patient before disclosure of this information.

Subpoenas and court orders are the other exceptions. A health care facility must present medical records for use in that particular court case.

Do I have to pay for the copies? 

If you want copies of your health care records for your personal files, there is a charge. If the copies are for your physician’s use, there is no charge. Attorneys and insurance companies must also pay a charge.

Our fees, which are set by the state of Missouri, can be attained by contacting the department at 573-882-3170.
