Request Access

If you are a provider who refers patients to University Hospital, MU Women's and Children's Hospital, Ellis Fischel Cancer Center or our clinics, you may request access to view the medical record for your referred patient.

If you work with a referring provider, you may request access to view the electronic medical record for patients referred by your provider to MU Health Care.

To request access, you must complete both the "Access and Confidentiality Agreement" and either the provider, or support staff, online access form:

For Referring Providers

Providers should complete all required information on MU Health Care "Referring Providers Online Access Form."

MU Health Care is strongly committed to ensuring compliance with all applicable laws, regulations and policies governing access to patient information and must uniquely identify each provider, and their support staff, who access their patients' electronic medical records. The National Provider Identifier (NPI) number has been established as the unique identifier and is required to obtain access to Referring Providers Online. Review and acceptance of the "Access and Confidentiality Agreement for Providers" is required before access can be granted to your patients' electronic medical records.

For security purposes, please do not include any patient-identifying information in your email correspondence. Fax and traditional mail are the preferred choices at this time.

Referring Provider's Support Staff

*Your sponsoring provider MUST be assigned access prior to support staff receiving access.

Complete all required information on the MU Health Care "Referring Providers' Online Access - Support Staff Form." 

The access form requires you to enter the clinic NPI or your primary provider's NPI. Your physician must have access to Referring Providers online prior to you receiving access.

What happens next?

After MU Health Care Medical Records receives your online registration, you will receive your access information via a confirmation email within 72 business hours. Then you may access your patients' electronic medical records, anytime, anywhere.
