The Senior Assessment Geriatric Evaluation (SAGE) Clinic provides a comprehensive assessment for older adults with medical, nursing, nutritional, and psychosocial needs.
Patients may be referred for SAGE evaluation by family members, physicians, community organizations, or by self-referral. The SAGE interdisciplinary team can address a number of issues that may be of concern to older persons and their families, such as:
- Memory concerns
- Physical limitations
- Emotional and behavioral changes
- Difficulties with independent living
- Alternative living arrangements
- Evaluation of medications

SAGE Clinic services may include physician referrals, physical or occupational therapy referrals, imaging, laboratory exams, and pharmacy services.
The SAGE Team
The Senior Assessment and Geriatric Evaluation clinic is designed to meet the needs and concerns of older adults. Our medical team is composed of a physician who evaluates the patient’s medical background through a detailed history and complete physical exam; a social worker who assesses the patient’s needs and connects the patient and care partners with community resources; and a dietitian who completes a nutritional assessment to evaluate the quality of the patient’s meal plan.
The findings and recommendations from the geriatric assessment are verbally shared at the conclusion of the SAGE visit. Written recommendations and contact information for the team members will also be provided during the visit.
The SAGE Evaluation
Once a SAGE appointment is scheduled, you will be mailed a packet of information from the SAGE team. This packet includes a release of records form and a three-page questionnaire. It is important that this information is returned to the clinic prior to your appointment in order to ensure a more comprehensive evaluation and assessment.
The SAGE evaluation takes approximately three hours. A family member or close friend should accompany the patient to the evaluation. It is also important to bring all current medications, including non-prescription medications, to the evaluation. Because of the length of the appointment, we recommend patients eat prior to the appointment. Patients are also welcome to bring a snack and any medication that you may need to take during the visit, such as insulin.
Payment for SAGE Clinic Services
Please bring your insurance card and a form of payment for your copay. Medicare and several supplemental insurances cover services at the clinic. Some people are in Medicare Advantage programs, also called Medicare Part C. It is important for you to look at your insurance card to see if you have this service.
If so, you must contact your insurance provider and ask if they will cover this appointment with the University of Missouri Health Care System. Sometimes coverage requires a referral from your usual physician. If the visit is not covered, you may be responsible for the entire cost of the visit.
