Nominating a Hero

To request a nomination form, either go to MyApps, Human Resources, 'Service Excellence Materials' or from Outlook:

  • Click on Tools.
  • Click on Forms.
  • Click on Choose Form.
  • Click on UMHS Service Excellence Materials.
  • Fill out the form and click Send.

Or you may submit a nomination electronically by e-mailing serviceexcellence@health.missouri.edu.

To nominate a hero, we ask that you describe how this person has succeeded at one, some, or all of the six Pillars described on our home page . Was there a specific action? Is there something the person said or did? Did this person do something special for you, a family member, friend, patient, or coworker? We are very eager to hear the story behind why you think this person is a Service Excellence Hero!
