Please fill out this form ahead of time and bring it with you to your appointment. This will speed up the process for you. If you don’t have the ability to print it out ahead of time, we will have copies at the vaccination site.

V-safe After-Vaccination Health Checker
V-safe is a smartphone-based tool that uses text messaging and web surveys to provide personalized health check-ins after you receive a COVID-19 vaccine. Through v-safe, you can quickly tell CDC if you have any side effects after getting the COVID-19 vaccine. Depending on your answers, someone from CDC may call to check on you and get more information. V-safe will also remind you to get your second COVID-19 vaccine dose if you need one.

ON THE DAY OF YOUR APPOINTMENT
Attire and behavior
When attending your appointment, please:
- wear short sleeves that are loose fitting.
- wear comfortable shoes for the walk from the parking lot to the Columns Club – wheelchairs are available for those who may need assistance.
- follow masking and social distancing guidelines.
- bring your photo identification and your completed consent form.
Arrival
Our vaccination site is located at Walsworth Family Columns Club at Mizzou’s Faurot Field, 600 E. Stadium Blvd. (See Map)
Because we are scheduling by appointment only, there is no need to arrive more than 15 minutes early. We have set up our appointment slots to ensure appropriate physical distancing and efficient patient flow. If you have an appointment, we have your dose saved for you!
The earliest we will check you in is 15 minutes before your appointment time. If you arrive more than 15 minutes before your appointment, please wait in your vehicle. We are trying to avoid people lining up outside the vaccination site in the winter weather. See the map below for parking areas.
Your appointment
At your appointment time, please come to the vaccination site entrance at the southeast corner of Faurot Field. Reminder: Do not come in more than 15 minutes before your appointment time.

Vaccine administration
As we continue to monitor our supply of critical PPE, including gloves, we opted to use a barrier bandage to administer COVID-19 vaccine shots so we didn’t need to use gloves that are in demand for patient care. The bandage goes on the injection site after it is sanitized, and the person administering the shot sanitizes his or her hands before and after administering the injection. The bandage re-seals after the injection, which prevents fluids from entering or exiting the injection site. Learn more.
