Bone Health Assessment

Weak bones can put you at risk for fractures. If you are concerned about your bone health, take this risk assessment survey.

  1. Are you a woman over age 65 AND have never had a bone density/DXA scan?
  2. Are you a man over age 70 AND have never had a bone density/DXA scan?
  3. Have you ever broken a bone from a minor fall after age 50?
  4. Do you have a family history of osteoporosis?
  5. Do you smoke cigarettes or use any nicotine?
  6. Do you drink three or more alcoholic drinks daily?
  7. Do you have any of these medical conditions?
    • Diabetes
    • Rheumatoid arthritis
    • Gastrointestinal diseases
    • Anorexia nervosa
    • History of gastric bypass surgery
  8. Have you used any of the following medications for more than three months?
    • Steroids
    • Acid reflux/heartburn/GERD medications
    • Antidepressants
    • Antiseizure medications
    • Medications for breast cancer or prostate cancer

If you answered “yes” to any question, you may be at risk for a fracture related to osteoporosis. Experts in our Bone Health Program can assess your risk of osteoporosis and suggest ways to improve your bone health.
