Chart Connect

Chart Connect is an online physician portal that allows you to securely access your patients’ MU Health Care electronic medical record.

A user ID and password are required to protect electronic patient information.

Within this web-based portal, authorized users will be able to:

  • Review your patients’ medical records, including progress, notes, testing results, medication orders and discharge summaries in real-time, 24/7
  • View results up to three years after your patients’ stay at MU Health Care
  • Authorize a colleague or other staff member to view your patients’ information

How to access Chart Connect as a provider

  • Complete a request form
  • Complete a request form and receive an application within 72 hours
  • Access your patients’ records 24/7

How to access Chart Connect as support staff

Use Chart Connect to obtain medical information about your patients who have received care at MU Health Care.

  • The sponsoring provider must be assigned access prior to support staff receiving completing a request form and receiving access
  • Complete a request form. You do not have to be a provider to complete this form 
  • Complete a request form and receive an application within 72 hours
  • Access your sponsoring provider’s patients’ records 24/7

MU Health Care is strongly committed to ensuring compliance with all applicable laws, regulations and policies governing access to patient information, and must uniquely identify each provider and their support staff who access their patients' electronic medical records. The National Provider Identifier (NPI) number has been established as the unique identifier and is required to obtain access to Chart Connect.
