New patient forms

Once you become pregnant, we will schedule follow-up appointments for early pregnancy monitoring and sonography before transferring your care back to your primary care provider.

WHEN TO SEEK INFERTILITY TREATMENT

We are happy to see patients at any stage of their journey.

However, as a rule of thumb, we define infertility as the inability to conceive after one year of frequent unprotected intercourse. For women older than 35 years of age, we encourage evaluation after six months of trying, and for women who are 40 years of age or older, we recommend expediting an infertility evaluation at your earliest convenience.

LGBTQIA+ FERTILITY SERVICES

Our team welcomes all sexual orientations and gender identities. It is our pleasure to guide and assist you along your family-building journey.

Single females and same-sex female couples can use donor sperm to build their families while undergoing either intrauterine insemination or in vitro fertilization. IVF would be the necessary treatment if one partner desires to donate the egg and the other partner wishes to carry the pregnancy.

We assist single males or same-sex male couples by linking them with egg donors and gestational carriers (surrogates).

We also provide fertility preservation services to transgender men and women before they transition. Through the use of cryopreservation, we can freeze their sperm or eggs and store them for future use.

ABOUT OUR EMBRYOLOGY LABORATORY

Our state-of-the-art embryology laboratory allows us to offer a full scope of fertility services to help you build your family.

Jaime Hogerty, PhD, is our lab director. Working in collaboration with your physician, she is responsible for growing embryos for IVF patients, performing preimplantation testing, preserving eggs and sperm and much more. If your fertility treatment involves our embryology lab and Hogerty’s expertise, she will attend your appointments to serve as another source of knowledge and support.
